Book this appointment if you need help filling in a PIP (Personal Independence Payment) form using your information. You will need to: - Have requested the form from DWP already - Have you National Insurance number with you on the video call - Bring any log-in information needed for online applications - Have a good internet connection (and a quiet location too!)
Appointment Policy
Only one form per booking. We aim to complete the application within the time allotted - if the form cannot be completed in this time you will need to book an additional Follow Up appointment of 30 mins to complete the remainder of the form with me. The success of the application is not the responsibility of Admin Frog and is down to the information I am provided with. I’m here to help you navigate the form and input the information you give me - not to answer the questions for you. If you require an interpreter because English is your second language you must arrange one yourself to help us in the call. You must have access to the internet, an email address, personal details (such as National Insurance) and any login information needed to complete an application online. Depending on the form you need help with - you may need to request this form from the proper authority before you attend our appointment - I cannot request this for you and your booking will not be refunded if this is the case. Please make sure you have requested all the required documents before attending the appointment. If you have any health support needs (deafness, anxiety etc) please let me know so I can support you as best as I can. You can cancel or rearrange an appointment up to 24 hours before your appointment. If you cancel before 24 hours you will receive a full refund however if you do not cancel at least 24 hours before your appointment we cannot give a refund. If you do not attend your appointment without canceling or rearranging or too late - we cannot give a refund. In some instances I will need your login information to help you complete the form - I do not collect or save this data and if you are concerned about your security please change you password after our appointment. If you are not happy with me using your login information we will be unable to help you complete your application as some forms will require me to login in on your behalf. We at Admin Frog cannot help you fill out physical forms - we can only help complete digital documents and applications. Any documents that require a signature will have to be printed and signed by you after our appointment (unless stated in your package) You are welcome to bring another person along with you in the appointment for support but please limit it only one person to make sure I can hear and understand you clearly and make this process as stress free as possible.
